Vitamin D deficiency is the most studied nutritional gap linked to knee pain, but it is not the only one. Low levels of vitamin C, vitamin K, and vitamin B12 may also contribute to joint discomfort, cartilage breakdown, or the inflammation that makes knee osteoarthritis worse over time. Understanding which deficiencies affect the knee and what they actually do inside the joint helps clarify why supplements alone sometimes fall short – and what options exist when they do.
Why Vitamins Matter for Knee Health
The knee joint depends on a range of nutrients to maintain cartilage, bone density, nerve function, and the control of inflammation. When any of these nutrients drop below adequate levels, the joint becomes more vulnerable to pain, stiffness, and progressive wear. Several vitamins have been studied specifically in the context of knee osteoarthritis, and their effects operate through different biological pathways.
Vitamin D and Knee Osteoarthritis
Vitamin D plays a central role in calcium absorption and bone mineralization, both of which affect the structural integrity of the bones that form the knee joint. Research has found that patients with low vitamin D levels tend to report higher pain scores on validated osteoarthritis measures compared to those with sufficient levels.[1] A cross-sectional study found that patients with early-stage knee osteoarthritis had significantly lower vitamin D levels than matched controls, and that lower levels correlated with higher pain intensity and greater physical disability.[2]
Vitamin D deficiency may also contribute to inflammation in joint tissue. Research has found an association between low vitamin D and elevated inflammatory markers in older adults, a population particularly vulnerable to knee osteoarthritis.[1] Evidence remains mixed across studies, but correcting a documented deficiency is generally considered a reasonable first step in supporting musculoskeletal health.
Vitamin C and Cartilage Structure
Vitamin C is a primary cofactor in the synthesis of collagen, the protein that gives cartilage its structural strength and ability to cushion the bones of the knee. Without adequate vitamin C, the body cannot properly form or cross-link collagen fibers, which can weaken connective tissue in and around the joint. Published case literature has documented knee pain and joint symptoms associated with vitamin C deficiency, with symptoms resolving after supplementation.[3]
Maintaining adequate vitamin C levels may help preserve the cushioning layer between bones, though supplementation alone will not rebuild cartilage that has already worn away. For patients whose cartilage loss is early or mild, supporting collagen production through proper nutrition is a reasonable complementary approach to other management strategies.
Vitamin K and Bone Mineralization
Vitamin K activates proteins involved in bone formation and calcium binding. A longitudinal study funded by the National Institutes of Health found that subclinical vitamin K deficiency was associated with a significantly increased risk of developing radiographic knee osteoarthritis and cartilage lesions on MRI over a 30-month follow-up period.[4] The mechanism appears to involve the carboxylation of proteins responsible for regulating bone and cartilage mineralization. Low vitamin K intake is particularly common among older adults, the population most affected by knee osteoarthritis.
Vitamin B12 and Nerve-Related Knee Pain
Vitamin B12 supports the myelin sheath, the protective coating around nerve fibers. A deficiency can cause neuropathic symptoms including pain, tingling, or discomfort that may be experienced in or around the knee joint even without structural damage. Low B12 is also associated with elevated homocysteine, a compound linked in some research to increased bone loss and joint inflammation. Patients with unexplained knee pain who have not responded to standard conservative care may benefit from having B12 levels checked as part of a broader metabolic evaluation.
How Vitamin Deficiencies Affect Knee Osteoarthritis
Knee osteoarthritis develops when the cartilage cushioning the joint gradually wears down, leading to inflammation, pain, and reduced function. Nutritional deficiencies do not cause osteoarthritis on their own, but they may accelerate the breakdown process or make the joint more susceptible to the effects of mechanical wear.
Vitamin D influences the activity of cells responsible for both bone and cartilage maintenance. When levels are low, the balance between tissue breakdown and repair may shift in ways that promote faster joint degeneration. Vitamin K affects how calcium is deposited in bone and cartilage, and deficiency has been associated in research with impaired cartilage health specifically in knee joints.[4] Vitamin C deficiency weakens the collagen matrix that gives cartilage its resilience. B12 deficiency may amplify pain perception through neurological pathways.
Together, these deficiencies can create an environment where the knee joint is less able to maintain itself, less able to manage inflammation, and more vulnerable to the progressive changes associated with osteoarthritis. Addressing identified deficiencies is one component of a broader approach to managing knee health.
Can Addressing Deficiencies Reduce Knee Pain?
Correcting a documented vitamin deficiency may help reduce some of the pain and inflammation associated with knee osteoarthritis, particularly in earlier stages of the condition. Research has found that vitamin D supplementation was associated with improvements in pain and function in patients with knee osteoarthritis, though longer studies are needed to confirm the full benefit.[1]
For patients whose knee pain stems primarily from structural damage, nutritional correction alone is unlikely to provide lasting relief. Once cartilage has worn significantly, the mechanical and inflammatory drivers of pain are no longer primarily nutritional. In these cases, the role of vitamins shifts from treatment to support – important for overall joint health but not sufficient to address the underlying damage.
A blood test through a healthcare provider can identify specific deficiencies. Supplementation doses and dietary changes should be guided by test results and a provider who understands the patient’s full clinical picture.

If knee pain has persisted for several weeks, worsens with activity, or limits daily tasks like walking and climbing stairs, it may be time for a specialist evaluation. A knee pain specialist can assess whether osteoarthritis is present, how advanced it is, and what combination of approaches – including addressing any nutritional gaps – may be most appropriate for the situation.
